When you apply for or use a wholesale prescriber account, we may collect:
- Professional identity: full name, NPI number, license number, DEA number (where applicable), specialty, and state of licensure
- Practice information: practice name, address, phone number, and fax number
- Contact information: professional email address and phone number
- Account credentials: username and password (stored encrypted)
- Prescription and ordering data: product selections, quantities, shipping addresses, and clinical notes associated with orders
- Payment and billing information processed by our payment processors on our behalf
- Communications you send us through forms, email, or support channels
Information We Collect Automatically
When you use the platform, our systems and third-party tools automatically collect:

- Service providers and vendors: We work with third-party vendors for cloud infrastructure, payment processing, analytics, and security. All vendors are contractually required to use information only as directed by us.
- Legal and regulatory disclosures: We may disclose information when required by law, regulation, court order, or valid government process.

HIPAA and Protected Health Information
Verum Health RX operates as a business associate to licensed prescribers under HIPAA. We do not use patient PHI for any purpose other than fulfilling the associated prescription order and as otherwise required by law.

Data Retention
We retain prescriber account information and order records for as long as necessary to fulfill the purposes described in this Policy, comply with applicable legal and regulatory obligations (including pharmacy and medical record retention requirements), resolve disputes, and enforce our agreements.
